Northwest Florida Health & Wellness Program

The Northwest Florida Health & Wellness Program is a community-embedded preventive health initiative designed to reduce chronic disease disparities among underserved and economically disadvantaged residents of Okaloosa, Walton, and neighboring counties. The program brings validated health screenings, culturally grounded education, and personalized care navigation directly into trusted community settings such as churches, schools, and civic organizations. By meeting people where they live, worship, and learn, NOML removes traditional barriers to care and strengthens long-term community health.
